Homespun Spindles offers instructions for spinning and plying (after you get some wool spun, go to the Andean plying bracelet link.). It is for a top whorl, but it really is the same for a bottom whorl. If you don't have a hook in the top of your spindle, just remember to put a slip knot in the notch before starting.
The Joy of Handspinning also offers a link for spinning and plying for a low whorl.
Hatchtown Farm offers this link on tuning a hook on the spindle.
Stasia's Wrist Distaffs contains a photo of a wrist distaff (holds the fibers while spinning.) You could maybe figure it out from the picture how to make one. Her links page looks pretty useful as well.
